    Greystone Home Health has an opportunity for a Medical Social Worker/MSW. The Medical Social Worker/MSW helps identify and provide for each patient's social, emotional, and psychological needs. Essential functions include but are not limited to developing a social history; social assessment and care plan which identifies status, pertinent problems and needs of the patient.

    Qualifications

    • Master's degree in Social Work or license and/or certification to meet state and/or federal requirements for the position.
    • At least one year experience in a home health environment is preferred.
    • Must have knowledge of state and federal guidelines.
    • Must have demonstrated skills in communication, documentation, assessment and social work methods techniques.
