    Greystone Home Health is currently looking for an RN Clinical Manager to join our team. Responsibilities will include:

    • Works within HCHB workflow structure and completes all daily tasks as directed by supervisor.
    • Utilizes SHP data analytics to guide review and promote accuracy of all OASIS assessments
    • Reviews and approves all agency referrals. Reviews available patient information related to case, including disciplines required, to determine home care needs.
    • Instructs and guides clinicians to promote more effective clinical performance and quality care delivery within the home care setting.
    • Assists clinicians in establishing measurable immediate and long-term therapeutic goals across all disciplines, prioritizing patient needs, and identifying care concerns.
    • Develops patient-specific plan of care for each patient receiving home care services including accurate medication list, homebound status, risk for hospitalization status, appropriate frequency based on care needs and patient-specific interventions, and goals.
    • Documents all change requests and makes necessary corrections to the collected OASIS data within the medical record. This may involve contacting the field RN/PT/OT/ST via telephone or e-mail to gain understanding of patient-specific assessment details.
    • Reviews and evaluates each case in coordination with Quality Assurance Coordinator to ensure accurate OASIS assessment data and ICD-10 coding. to facilitate timely plan of care approval.
    • Holds weekly team case conference meetings with field clinicians and agency personnel facilitate appropriate coordination care.
    • Conducts quarterly assigned record reviews and communicates findings and recommendations to Director of Clinical Services and appropriate organization personnel.
    • Performs other duties and activities as delegated by the Director of Clinical Services.

    Qualifications

    • Two years management or supervisory experience in health care setting, preferably home care.
    • Six months of Home Health field experience
    • Must have proof of current CPR certification, driver’s license, and car insurance.
    • Able to perform duties in accordance with generally accepted standards of clinical practice, the Medicare Conditions of Participation, Federal and State of Florida statutes as well as ACHC accreditation standards.
    • Assists Regional Director of Clinical Outcomes in meeting ACHC, federal, state and local regulations and standards.

     Education:  Currently licensed as a Registered Nurse (RN) in good standing with the State Board in which he/she practices. BSN preferred.
