    Responsibilities

    Cardiopulmonary Coordinator

    The Cardiopulmonary Coordinator Position (CPC) will maintain direct oversite of clinical education, office process, and growth/ improvement metrics for their assigned location for the Cardiac/Pulmonary Division. 

    Essential Job Functions/Responsibilities

    Clinical

    • First-week (7 day) Cardiac welcome call to be performed and documented in patient chart
    • 30-Day Mid Eval patient home visits to be performed and documented in chart
    • Clinician ride-alongs to ensure competencies, education and clinical support
    • Clinical assessment to be performed for high acuity Cardiopulmonary patient only when all resources have been exhausted                                                                 

    Administrative

    • Tracking all Cardiopulmonary Patients (audit of Division tenets, tracking tools, patient oversight/needs)
    • Root Cause Analysis for all Cardiopulmonary Patients readmitted to the Hospital within first 30 days
    • Vetting of daily referrals and product coding appropriately in system
    • Active in all Case Conference meetings to follow up on existing Cardiopulmonary Patients
    • SOC, Hospitalizations, ER visits, plan for Recert/Discharge, any concerns related to current patients
    • Orientation on Cardiopulmonary for all new employees
    • Education and in-service opportunities
    • Active in all Business Development meetings; scheduling of Planned sales calls
    • Cardiopulmonary Chart Audits to ensure the tenets are being followed
    • Assures compliance with all local, state and federal laws regarding licensure and certification of organization personnel and, maintains compliance to the ACHC Home Care standards
    • Stays informed about changes in the field of nursing and home health care; shares information with appropriate organization personnel
    • Monthly Program /Office metrics:
      • KPI, Clinical outcomes, SHP/Einstein, PDGM ROI
      • Census, CMW, R/C, VPE for Cardiopulmonary Specific Patients
      • Trending Reports

    Sales

    • Minimum of one sales call with each sales associate monthly or as needed in targeted accounts
    • Relationship building with referral sources (Physician’s, Hospital case managers, ACO)
    • Outcomes/ Patient updates/RCA for sales associate to provide for Referral source
    • Assist sales in Cardiopulmonary educational ideas for referral sites

    Qualifications

    1. Registered nurse with current licensure to practice professional nursing in the state
    2. Cardiovascular and Pulmonary experience as a Registered Nurse for Minimum of Three (3) years
    3. Degree in Nursing from an accredited program by the National League for Nursing
    4. Demonstrated ability to supervise and direct professional administrative personnel
    5. Ability to market and deal tactfully with customers and the community
    6. Has excellent observation, verbal and written communication skills
    7. Must have proof of current CPR certification, driver’s license, and auto insurance
    8. Knowledge of business management, governmental regulations and ACHC standards
