    MDS Coordinator Float

    Miami/South Florida Region

    Daily travel; some overnights required


    We are looking for a high-energy, organized, focused, and results-driven MDSC to add to our team. Duties and responsibilities include providing MDS completion support to various facilities in the South Florida region as needed each day, with short- and long-term assignments at our various facilities. The ideal candidate will live near the Miami area and be available to travel daily to various Greystone skilled nursing facilties in the South Florida region.  This position reports to and takes direction from the Corporate Director of Clinical Reimbursement/ Utilization Review.


    Along with having at least 2 years of full-time MDSC experience, a basic understanding of PDPM, and experience with PCC, responsibilities of the MDS Coordinator include but is not limited to overseeing and coordinating the development and completion of the member assessment (MDS) in accordance with current Federal and state regulations, coordinating and leading the development of a comprehensive member assessment and care plan for each member as chairperson of the community Interdisciplinary Plan of Care team, ensuring the face validity of all Minimum Data Sets before electronic submission and combine the timely electronic submission of all MDS, and conducting a monthly review of a random sample of the MDS to ensure accuracy and internal consistency with the medical records.


    • Must be an active Licensed Practical Nurse (LPN) or Registered Nurse (RN) in good standing licensed by the state of Florida. RN preferred. 
    • Must have at least 2 years’ experience as a Nurse.
    • Must have knowledge in PPS assessment as well as OBRA compliance.
    • Demonstrates clinical assessment skills at the level necessary to meet the job requirements.
    • Ability to make independent decisions and follow through with limited supervision.
    • Knowledgeable and understanding of state and federal regulations and guidelines governing the practices of the facility.
    • Effective interpersonal skills and the ability to work cooperatively with an interdisciplinary care team.
    • Able to relate positively, favorably and cooperatively with others, including, members, family members, associates and personnel of outside agencies and organizations.
