    Park Meadows Health & Rehab Center, a 154-bed facility, managed by Greystone Healthcare Management, currently has an opening for an Abuse Coordinator.   As an Abuse Coordinator you are responsible for the development and implementation of an effective Risk Management/Quality Assurance Program that includes oversight, training, continued education and support to all facility personnel. You will conduct a complete investigation and analysis of the risk, with subsequent root cause determination and the development and execution of a facility response plan related to the findings. You will also assure compliance with state and federal reporting guidelines.


    Duties & Responsibilities:

    • Directly reports to the Center's Executive Director.
    • Attend/ Participate in monthly QAPI Committee.
    • Track and trending for root cause analyzes to determine problem areas and implement performance improvement plan.
    • Participates in select meetings relating to assessment and provision of patient/resident care including Standards of Care.
    • Develops and implements an educational program on Risk Management and Quality Assurance Performance Improvement for all personnel that occurs during new employee orientation and at least annually for all employees.
    • Leads investigation and development of corrective action plans.
    • Develops and submits reports to state and federal agencies as required by law.
    • Reviews resident/family complaints and grievances as responds with investigations and follow up as indicated
    • May be asked to participate in patients plan of care.
    • Communicates with patient/ responsible party
    • Has access to all patient/resident medical records including paper and electronic files.
    • Investigates and analyzes actual and potential risks with the center, such as;
    • Unplanned return to hospital visits
    • Unusual events/incidents
    • Multiple or severe occurrences, such as;
    • Elopement
    • Fall resulting in injury or multiple falls
    • In house acquired pressure ulcers
    • Weight loss and/or dehydration
    • Chronic or intensive pain
    • Incident(s) requiring transfer to hospital
    • Unexpected/unexplained death
    • Acute change in resident condition
    • Significant decline in condition
    • Significant risk choices (such as Smoking, eating outside of prescribed diet) 
    • Review of facility practices
    • Facilitate review of Standards and Guidelines across disciplines, and provide teaching training and training related to facility identified needs, to include annual risk training


    • Professional knowledge of Risk Management
    • RN preferred
    • Working knowledge of Federal and State regulations/laws
    • Working knowledge of Joint Commission regulations
    • 2+ years of Risk Management experience in skilled nursing/long-term care preferred
    • Certification of completion with an approved risk management program
    • Strong computer skills – Microsoft Office Suite and a basic understanding of relational data bases
    • Working knowledge of electronic health records
    • Excellent interpersonal, communication, presentation and analytical skills

    • Teamwork and customer focus skills required
